THREE WAYS TO PURCHASE A MEMBERSHIP:

PAY ONLINE: click the link above to pay for one or two memberships, print and complete the membership form (link below), and return the form to the membership table at our next Clubhouse 5 event. If you are unable to print the form, we will have one for you at the membership table. Membership cards will NOT be mailed, you must pick up your membership card at any Clubhouse 5 event.

PAY AT CLUBHOUSE 5 EVENT: Print and complete the membership form (link below), and return the form to the membership table at our next Clubhouse 5 event and pay with a check, cash or credit card. If you are unable to print the form, we will have one for you at the membership table. You will receive your membership card immediately.

PAY BY MAIL: Print and complete the membership form (link below), mail the form along with your personal check to the address indicated on the form, and pick up your membership card at any Clubhouse 5 event. Membership cards will NOT be mailed, you must pick up your membership card at the any Clubhouse 5 event.
